During this session, the client and counselor get to know each other, identify areas you want addressed, and agree on counseling goals. You will learn more about the counseling process and what to expect.**

Client and counselor work on above stated goals. Assessments may be administered to check for progress.

Evaluation for ADHD

Book Now

ADHD evaluation consists of an interview and testing. Questionnaires will be completed by client, parent (when applicable) and teachers (when applicable). Additional session fees described above apply.

* Client paperwork will be e-mailed to you for completion before your appointment. If you are unable to do this, please arrive 15 minutes before scheduled appointment to complete in the office. J Piazza Counseling is on limited insurance plans. Apologies for any inconvenience.

** If client is a minor, the initial intake session is for parent/guardian only. The child will not be present for this session. Guardianship papers are required for counselor to meet with minor child.
